Health Insurance:

I am not currently in network with any health insurance companies. If you have out of network massage therapy benefits (call your insurance company to find out if you aren’t sure), I can provide you with an invoice/superbill that you can submit to your insurance for reimbursement. For this option I require payment at the time of service.

Injury Treatment:

I accept and can directly bill the following insurances for injury treatment massage therapy:

Personal Injury Protection (PIP) for Auto Injuries.

L&I and OWCP for work injuries.

*Please note, if you were in a car accident and you don’t have personal injury protection but would like to be treated for your auto injury, you will need to pay at the time of service for your appointments and I can provide you with an invoice/superbill and chart notes so you may pursue reimbursement later from your settlement.
